Consent Form

ReGenesis Youth UK CIC (RYUK) will be taking pictures and video footage of all activities including the participants. This will be used as feedback, as part of our evaluation process, in marketing and advertising materials, in our publications, on our website and social media platforms, on our funders and partners website and social media platforms and for the children's and young peoples viewing . 

You / your child will not be identified by name in any publicised materials without consent.

If you do not want yourself / your child to be filmed or photographed that may mean that they may not be able to participate in some of the activities and events.

By completing and submitting this form you are giving consent for you / your child to be filmed and photographed in our activities and workshops.
